California Healthy Kids Survey

The California Healthy Kids Survey (CHKS) is the largest statewide survey of resiliency, protective factors, risk behaviors, and school climate in the nation.  Annually, 5th grade students are invited to take part in the CHKS.  This survey helps promote better health and well-being among our youth, improve the school learning environment, and combat problems such as drug abuse and violence.  The CHKS is a voluntary survey, and active consent permission slips are sent home to families of 5th grade students prior to the survey being administered.  This year the CHKS survey administration window is from November 1, 2022 through December 22, 2022.  Parents/guardians can review the questions of the CHKS in the office of their child's school, or by downloading the 2022-23 California Healthy Kids Survey from the chks.wested.org website.
