Below is the list of forms required to be submitted on MAZE Day. Parents/legal guardians will receive the Registration Packet as early as May 16, Open House at Gomes. For those that did not pick up, the packets will be sent to their residence a week prior to the scheduled MAZE Day, Tuesday, August 29, 2017.
- STUDENT EMERGENCY INFORMATION FORM (1 white half sheet index, back-to-back)
- DISASTER RELEASE INFORMATION FORM (3-ply document, do *not* separate these forms after completing)
- FUSD TECHNOLOGY FORM (1 white sheet, back-to-back; not included in packet but *must* be printed online at FUSD website http://www.fremont.k12.ca.us/handbook, FUSD Parent-Student Handbook 2017-18, completed and submitted on Maze Day)
- OPTIONAL OPT-OUT FORM DIRECTORY INFORMATION (not included in packet but can be obtained online at FUSD website http://www.fremont.k12.ca.us/handbook, FUSD Parent-Student Handbook 2017-18)
